Health Declaration

I, the undersigned, hereby declare that:

• I am aware that participating in a golf competition is a sporting activity that involves physical effort, and that the responsibility to ensure my medical fitness for participation rests with me.

• I declare that my health condition allows me to participate in the competition, and that I do not suffer from any illness, medical limitation, or health condition that may endanger me or others during the competition.

• I declare that I have not been prohibited by a physician from engaging in physical activity, and that I have not received any medical instruction to refrain from participating in sporting or competitive activities.

• I undertake to immediately notify the representatives of the Israel Golf Association, prior to the competition or during it, of any change in my health condition or any relevant medical event.

• I am aware that, in accordance with the provisions of the Sports Law and the Sports Regulations (Medical Examinations), I may be required to present a valid medical certificate, including required tests such as an ergometry test according to age and subject to the law.

• I confirm that my participation in the competition is at my own responsibility, and that the Israel Golf Association and/or the competition organizers are not liable for any medical damage, injury, or health event that may occur to me during the competition and/or in connection with it, except for liability that cannot be waived under law.

• Participants over the age of 35: By signing this declaration, I confirm that I possess a valid ergometry test performed within the past 12 months, that its results are normal, and that I have not concealed any material medical information related to my fitness to participate in the competition. The responsibility for performing the test, ensuring its validity, and uploading it to the system together with this health declaration rests solely with me. I am aware that failure to upload the test as required is solely my responsibility, and that the Israel Golf Association, its managers, and its officers may rely on my declaration and on the documents I have uploaded, all subject to applicable law.

Please note: uploading an ergometry test is mandatory for players over the age of 35.
