Health Declaration – Participation in Association Competitions

I, the undersigned, hereby declare that:

1. I am aware that participating in a golf competition is a physical activity that involves physical effort, and that it is my responsibility to ensure that I am medically fit to participate.

2. I declare that my health condition allows me to participate in the competition, and that I do not suffer from any illness, medical limitation, or health condition that may endanger myself or others during the competition.

3. I declare that no physician has prohibited me from engaging in physical activity, and that I have not been instructed to refrain from participating in sports or competitive activities.

4. I undertake to immediately inform the representatives of the Israel Golf Association, prior to or during the competition, of any change in my health condition or any relevant medical event.

5. I am aware that, in accordance with the Sports Law and the Sports Regulations (Medical Examinations), I may be required to present a valid medical certificate, including any required tests such as an ergometric test, depending on age and as required by law.

6. I confirm that my participation in the competition is at my own risk, and that the Israel Golf Association and/or the competition organizers are not responsible for any medical damage, injury, or health event that may occur to me during or in connection with the competition, except for liability that cannot be waived under the law.
